Foxbury Care – Terms & Conditions
- The provision of routine examinations and hygiene visits are subject to receipt of the required consecutive monthly payments.
- Any treatment required where payments have not been received may be refused.
- All Foxbury Care plans are based on a 12 month contract and will be renewed automatically for a further 12 months unless otherwise notified.
- You must pay sufficiently in to the plan before you can claim treatment. For example, an adult on Plan 1 would be able to claim their next checkup or hygiene appointment 10 months in to the 12 month term. For an adult on Plan 2, you would be able to claim at 4 months in and 10 months in respectively.
- You must also be registered as a patient at our practice before you can use the plan and a new patient exam is mandatory before you can be registered as a patient. Your dentist may advise you which plan is best suited for you at that current time.
- Switching between plans can be organised at the end of every 12 month term in collaboration with your dentist.
- The use of X-rays is included in all plans but to be used at the dentists discretion to minimise the impact of radiation when not absolutely required.
- A scale and polish is included in your plan either by hand, or using aerosols. This does not include airflow which is charged for seperately.
- Treatment for periodontal disease is not covered on the plan and will be provided as a seperate treatment plan by your hygienist for consideration.
Our care plan payments are taken by Go Cardless which operates like a direct debit to your chosen bank account. More information can be found here.
Family Plans
- The cost per month varies by the adult plan you are on and the age of children on the plan.
- The price of the premium will increase as children get older and move between bands.
- All kids under 7 years old are free and will not increase the premium.
- Kids will receive the same amount of check-ups as on the associated adult plan and must come with a parent.
- Two adults in one family can be on the same plan only if they are on the same plan interval. mix match of intervals (Plan 1 or Plan 2) is not supported.
